Registration & Requirements

Any athlete that plans to participate at Severance High School must have the following completed:

• Athletic Registration view instructions and Fees
• Up-to-date sports physical using the required CHSAA physical form ( English) | CHSAA physical form ( Espanol )

Athletes will not be allowed to participate without these materials.

Student Accident Insurance

• Weld RE-4 students can purchase accident insurance through K & K Insurance. They offer affordable plans for student/athletes in need of coverage.
• Insurance is required for our student-athletes to participate in the Weld RE-4 district.
• Optional Insurance Opportunities (English Application | Spanish Application)
